The whole consultation, written the way a GP writes it

A primary care consultation covers a lot in ten minutes. Cognivolt writes it up as a complete clinical history, checks what you prescribed against the patient’s allergies and current medicines, and keeps it in a chart that remembers the last visit.

Who it is for: GPs, family medicine and internal medicine physicians.

Complete history

Presenting complaint through systems review, examination, investigations, impression and plan, in that order.

Allergy & interaction checks

Checked against fixed, versioned reference tables, not a language model’s opinion.

What changed

Follow-up visits are compared with earlier ones, so a change is flagged instead of re-asked.

A systems review is never invented

The systems review records the positives and relevant negatives you actually asked about, and the examination records what you actually examined. A normal finding you did not check is never written in.

Red flags outside the presenting complaint

If something serious comes up that is not what the patient came in for, the note records it and the onward workup it needs, rather than leaving it out because it was off-topic.

Coding that matches the visit

After you confirm the diagnosis: primary and supporting ICD-10 codes, a visit code at the complexity actually documented, and a note on which HCC categories the documentation supports. Outside the US, the coding system your country uses, or nothing where it has no standard one.

Frequently asked questions

Will Cognivolt write a normal examination I did not do?

No. The examination records only what you examined and described, and the systems review only what you asked about.

Does it check prescriptions against allergies and current medicines?

Yes, against fixed, versioned reference tables rather than a language model’s opinion.

Can I get the note into my existing EHR?

Yes. A browser extension pastes the approved note into whatever EHR you already have open.
