What is a mental state examination?
A structured clinical assessment of how a patient presents in the moment — appearance and behaviour, speech, mood and affect, thought process and content, cognition, insight and judgment. It's a standard part of psychiatric and mental health documentation that a general medical scribe does not produce at all, since it isn't a transcript of what was said so much as a clinical read of how it was said and presented.
