Cognivolt — AI clinical documentation, decision support and EMR
Cognivolt listens to a clinical session and writes it up as a SOAP, DAP,
BIRP or History note, with a mental state examination and a structured
risk assessment, while the session runs. Nothing downstream of a
diagnosis — no ICD-10 or CPT code, no clinical guideline, no
psychoeducation content — is generated until the clinician confirms the
diagnosis themselves.
What it generates during the session
- The note, in SOAP, DAP, BIRP or History format, or your own custom template
- A structured mental state examination
- A risk assessment (for example low, moderate or high), reviewed and signed by the clinician
- Ranked differential diagnosis considerations with supporting evidence
- Longitudinal insights pulled from a patient's prior visit history on follow-up sessions
After the diagnosis is confirmed
ICD-10 and CPT coding suggestions, relevant clinical guideline references,
and a psychoeducation guide for the patient — generated for the confirmed
diagnosis and specialty context of that session, never before.
Built for more than one specialty
Psychiatrists, PMHNPs, psychologists, psychotherapists, counsellors, GPs,
family and internal medicine physicians, cardiologists, neurologists,
dermatologists, gastroenterologists, nephrologists, orthopaedic and
general surgeons, and paediatricians are all supported today, each with
specialty-tailored note structure and clinical language.
Also an EMR and clinical decision support layer
Documentation, decision support and the patient record share one
patient history — automatic speaker separation for in-person and
telemedicine sessions, allergy and drug-drug interaction checks against
fixed reference tables, referral letters and discharge summaries drawn
from the patient record, and FHIR export.
Security and privacy
Cognivolt is built on HIPAA-eligible infrastructure with a Business
Associate Agreement (BAA), is built to be GDPR compliant, uses
end-to-end encryption so not even Cognivolt's own staff can read patient
records, and does not retain session audio once a note has been
generated from it.

Questions clinicians ask
Does it write BIRP notes?
Yes. SOAP, DAP, BIRP and History are all available, chosen per session.
Will it code a diagnosis I did not confirm?
No. Nothing downstream of a diagnosis is generated until the clinician confirms it.
Is session audio stored?
No. Once a usable summary has been generated from a session, the raw audio is permanently deleted.
Does it work for telemedicine?
Yes. Telemedicine mode captures the clinician's microphone and the patient's system audio as two separate, diarized streams.
