The History note for a mental-health clinician is a psychiatric intake: past psychiatric history (previous episodes, hospitalisations, ECT), family psychiatric history, a social history that covers development, relationships and forensic history, and substance use as its own heading rather than a line inside the social history.

The mental state examination is generated with the note

Appearance, behaviour, speech, mood, affect, thought form, thought content, perceptions, cognition, insight and judgement, each written from what the session actually showed. A domain the session gave no information on says so instead of being filled in as normal.
