    The note records what was said in the consultation. An examination that was not done is never written as normal.

    Every dose, measurement and scale score in the note is checked against what was actually said, by checks that run outside the AI. A number nobody said is caught before it reaches you.

  3. A final audit layer

    Before you see anything, an AI audit agent reviews the note, the MSE and the differential considerations against the consultation, including whether something the patient denied was written as present.

  4. You approve
